Privacy Notice and Consent for Champion site (AE, MI and PQC)
AstraZeneca (the company) would like to respond to your requests regarding AE reporting, MI (medical information inquiry), and PQC (product quality compliant) through the CHAMPion site, an integrated customer request processing site.
The items of personal information required for each report and inquiry, purpose of processing and retention period are as follows. Please take a close look and show your agree on each consents below.

Consent to collect and use personal information
Adverse Event
Purpose of collection and use: Collects abnormal case information and reports to the regulatory authorities and headquarters as a post marketing safety management.
Information to collection and use: Name (if necessary), date of birth, gender, height, weight, contact information (if selected), occupation. (to find whether HCP or not).
Retention period: 15 years after the end of the item license.
Medical Information
Purpose of collection and use: Reply to your medical information inquiries.
Information to collection and use: Name, contact information (phone number or mobile phone number), E-mail (if necessary), and Occupation (for the purpose of verifying health care professionals).
Retention period: 6 years from the date of consent.
Product Quality Complaint
Purpose of collection and use: Receiving complaints of AZ products, sending new product as an exchange and reply with investigation reports.
Information to collection and use: Name, Contact information (phone number or mobile phone number), Work address and email address (if necessary).
Retention period: Up to 6 years from the date of consent.
